Community Health Needs Assessment

FMCH partnered with the Lee County Health Department and Keokuk Area Hospital to conduct a Community Health Needs Assessment. This consisted of a community wide survey and town hall meeting.  The following health needs were identified as 2017-2019 Community Health Needs Goals:

  • Promote healthy lifestyles/healthy behaviors for all. Reduce chronic disease.
  • Improve mental health/behavioral health issues/access to care. Increase access to service.
  • Reduce transportation barriers of our workforce an access to care.  Increase usage and reduce barriers.
  • Improve communication, provide education and motivate our population for change.  Community outreach and education.
