Scholarship Applications

Fort Madison Community Hospital facilitates the disbursement of two scholarships: Kathleen Canella Family Scholarship and the FMCH Auxiliary Scholarship. Both Scholarships will be awarded to students pursuing a degree in a medical profession. An overview of the criteria is below. For a full explanation and requirements, please click on the individual scholarship. Please make sure you read the necessary criteria carefully. While the application can be completed online there are additional documents required for both scholarships that must be sent in separately for your application to be complete.
 

Kathleen Canella Family Scholarship

The Kathleen Canella Family Scholarship will be awarded to students graduating from Holy Trinity Catholic High School and Fort Madison Community High School.  Non-traditional students will also be considered. Students must be pursuing a degree in a medical profession other than physician. Points for consideration include grades, leadership, character, community activity and financial need.
 
Kathleen Canella Family Scholarship Applications are due by the 1st of April.

FMCH Auxiliary Scholarship

The FMCH Auxiliary Scholarship will be awarded to students graduating from any area high school within the market area of FMCH. Non-traditional students will also be considered. Students must be pursuing a degree in a medical profession. Points for consideration include grades, leadership, character, community activity and financial need.
FMCH Auxiliary Scholarship Applications are due by the 1st of April.

Tracey Howe Scholarship

The Tracey Howe Scholarship is available for nursing students attending Southeastern Community College in Keokuk or West Burlington. The FMCH Foundation provides two $1,000 awards for scholarship assistance to new or continuing nursing students. This application must be completed online and will not be considered until all required items have been recieved. 

Additional requirements:

  • a copy of applicant's college transcript
  • two letters of support- one personal and one professional
  • a copy of the EFC from FAFSA

All items may be emailed to Carrie Green at cgreen@fmchosp.com or mailed to FMCH Foundation, PO Box 174, Fort Madison, IA 52627. Complete applications are due by April 2.

Iowa Hospital Association Scholarship

The Iowa Hospital Education and Research Foundation (IHERF) Health Care Careers Scholarship program began in 2005. IHERF is a wholly owned subsidiary of the Iowa Hospital Association. IHERF is a charitable nonprofit and exempt from federal income tax under section 501(c)(3) of the IRS code. IHERF has served many functions over the years – the current main function is the oversight of IHERF’s health careers scholarship fund. The goal of this program is to fill health care positions that currently have a significant number of openings across the state and to recruit students with the opportunity to obtain significant scholarships to help cover the costs of health care education.
