Diabetes Education
The Fort Madison Community Hospital Diabetes Self-Management Program is designed to help
diabetic individuals enhance their health by learning to effectively manage Diabetes. The
FMCH Diabetes Self-Management program has The American Diabetes Association Certificate of
Recognition for meeting the National Standards for Diabetes Self-Management Education.
The program also has State of Iowa Certification from the Iowa Department of Public
Health.
HOW does our program work?
- It begins with a private appointment with a diabetes educator and/or dietitian to assess your individual needs and assist you in personal goal setting.
- Next, an 8-hour group class, offered monthly, provides in-depth information about diabetes.
- It continues with private sessions with diabetes educators to monitor your progress until your goals are met.
- Upon completion, you'll graduate with improved diabetes control and increased confidence in your ability to manage your health.
