Provider Based Billing

On October 1, 2016, Fort Madison Community Hospital (FMCH) will be transitioning all of our clinics to Medicare Provider Based Clinics.  This transition will only affect the way you will be billed for the health care services you receive in the FMP&S Clinics. 
The most important thing we want you to know is that you can expect the same great care from your existing providers; only if you have Medicare will your billing change.  From October 1st, 2016 onward, every time you receive care from one of our FMP&S providers, you will receive a statement from FMCH that includes a line item charge for your health care provider/professional services and a line item charge for the facility/hospital charges.  Your insurance plan coverage will be applied to both bills and the remaining balances will be billed to you – one bill for the provider charge and one bill for the facility/hospital fees. Provider Based billing will not change your covered services but it may affect your co-payments, deductibles and co-insurance.  We encourage you to check with your insurance company to find out which portion of your charges will be covered under Provider Based billing.
The new system will require adjustments for all of us, and we expect that you may have questions.  Please take a few minutes to review the list of frequently asked questions included. We encourage you to contact our Patient Financial Services department at 319-376-2123 if you have any further questions or concerns.  Likewise, FMCH has SHIIP counselors onsite who can help you evaluate your Medicare coverage and the possible need to pick up a Supplemental policy to offset any co-payments or deductibles.

Frequently Asked Questions about Medicare Provider Based Clinics


What does Provider Based Billing mean?

“Provider Based” refers to the billing process for services received in a hospital (FMCH) owned medical practice (FMP&S) that is clinically integrated departments of the hospital. Simply put these are terms Medicare uses to describe outpatient clinics that are actually part of the hospital. This is the national model of practice for healthcare organizations that provide both hospital and physician clinic services to their patients. This change only affects our patients covered by Medicare.


Why is FMCH making this change?

Fort Madison Community Hospital is taking proactive steps to ensure high-quality care remains available to all of our patients.  We strive to maintain a high level of physician services are made available locally.  By converting to the Provider Based billing model, FMCH is ensuring it is reimbursed fairly for services it provides.


What will be different?

You will continue to be able to see your provider of choice at FMP&S and can expect the same level of quality care you have always received. Will this affect my co-pays, co-insurance or deductibles?

Depending on which clinical service is provided, there may be additional out-of-pocket expenses you may be responsible for.  Your insurance plan coverage will be applied to both bills and the remaining balance will be billed to you. We recommend that you consult directly with your insurance company to find out which portion of your charges will be covered under the new system.


What if I have secondary/supplemental insurance coverage?

Co-insurance and deductibles may be covered by a secondary insurance policy.  Again, we encourage you to contact your insurance company and ask if they cover facility charges or provider based billing. Make sure to ask what percentage is covered and verify your benefits.  If you are interested in acquiring a secondary or supplemental insurance policy, an appointment with a SHIIP counselor at FMCH can be set up by calling 319-376-2249.

What are facility/hospital charges? I did not go to the hospital; I went to see my doctor…

The FMP&S Clinic where you received services is an outpatient department of Fort Madison Community Hospital.  FMCH is responsible for the cost to operate and maintain the clinics; therefore, based on Medicare regulations, it is an extension of the hospital.
