Home Health & Hospice Care

FMCH Home Health & Hospice will continue to provide in-home health care services that include medical care, personal care and rehabilitation therapy while adding hospice care. The focus will remain on teaching clients to care for themselves, manage their health challenges and remain safely in the comfort of their own homes.  By adding hospice care, these professionals will be joined by a chaplain and social worker to provide continuity of care for chronically ill, terminally ill or seriously ill patients.  Working together as a care team, they will be able to develop a personalized plan of care to help these patients and their families through this difficult journey. 
  • Monday-Friday 8:00 a.m. – 4:30 p.m. 
  • A Registered Nurse is on call 24 hours a day to meet the health care needs of our clients. 
FMCH Home Health Care understands that there really is no place like home. As part of the FMCH family of health care professionals, they uphold the same high standards of care that you have come to expect from FMCH; allowing them direct access to health care resources and support. 
FMCH Home Health Care Services is made up of a dedicated group of professionals who realize we are guests in your home and it is our goal to work with you and your family to create and provide you the best plan of care. The professional staff of FMCH Home Health Care is ready and able to teach you to care for yourself, manage your health challenges and ensure your safety and comfort. 
These professionals include the following: 
  • Physical Therapy 
  • Registered Nurses 
  • Nurse Aides

Provided by a Registered Nurse

  • Medication management 
  • Assisting with access of community and funding resources 
  • Disease management and education 
  • Evaluation visits 
  • Assessment of unstable and/or new medical conditions 
  • Maintenance care 
  • Post-op surgical cares; dressing changes, wound care, drainage tube care and feeding tubes 
  • Blood draws 
  • Flu shots 
  • IV's 
  • Ostomy pre-op and post-op teachings and care 
  • Orthopedic post-op trauma care 
  • General post-op care 
  • Hospice Care


Provided by Home Health Aids
  • Assistance with walking, grooming, bathing, dressing and nail care 
  • Incidental housekeeping and meal preparation 


Provided by Professional Therapists
  • Physical, Speech, or Occupational Therapy to recover from illness or injury

Our services can help anyone, including: 
  • Discharged hospital or nursing home patients needing follow-up care. 
  • Patients with diabetes, muscle or nerve disorders and heart, kidney or respiratory disease.

  • Terminally ill patients who choose to remain at home. 
Family caregivers of Home Health Care recipients also benefit from our services, as we can share the responsibilities of care and lend a helping hand or shoulder.
